    General Liability Certificate of Insurance Upload (COI)

    To become a Certified Vendor for CheckSammy, you must meet our Minimum Insurance Requirements and have CheckSammy named as the certificate holder. To view a sample of a Certificate of Insurance with the required verbiage, click on Sample COI. Be sure to note the necessary limits on the COI.

    Please upload your current W9 with completed Tax Identification details. If a blank W9 is needed it can be downloaded here.

    Photos of Equipment Used

    Please provide at least 3 photos of your truck / trailer/ open tops that are used in your business.
