    • To become a Certified Vendor for CheckSammy, you Must meet our Minimum Insurance Requirements and have CheckSammy named on the COI.

      To view a sample of a Certificate of Insurance with required verbiage, click on Sample COI. Be sure to note the necessary limits on the COI. If your COI is not available at the time of this application, please email it to [email protected] when it is available. Please note that you will not be permitted to work for CheckSammy until the required documentation is received and approved.     • Would you please upload a W9 and the following compliance documents to get started, General Liability, Commercial Auto, Workers Comp, Occupational License, or Business Permits. Dependent upon specific locations, additional documentation and compliance may be needed to further our stringent vetting process.
